Optomap / Macula Wellness Scan Consent Form

At Eyecare of Grand Lake, we are committed to providing the highest level of eye care. As part of your comprehensive eye exam, we offer an advanced retinal imaging test called the Optomap / Macula Wellness Scan.

This scan provides a high-resolution 200-degree panoramic image of the inside of your eye, allowing our doctors to evaluate the health of your retina and macula without the need for dilation in most cases. It can help detect or monitor conditions such as:

• Diabetic changes
• Hypertension effects
• Retinal holes, tears, or detachments
• Macular degeneration
• Other diseases that may not produce symptoms early

The scan is quick, painless, and non-invasive, and the image becomes a permanent part of your medical record for future comparison.
